Patient Rights and Responsibilities
Rights
As a patient of English Plastic Surgery Center & Surgi-Spa you have the right:
- To be provided with impartial medical care regardless of race, creed, sex, national origin, and religion.
- To be provided considerate, respectful and safe care while maintaining your personal dignity.
- To be interviewed and examined in a private room with someone of the same sex with you.
- To know your diagnosis, treatment and any known prognosis. Your medical and personal records will be treated confidentially to the extent protected by law.
- To make decisions involving treatment and consent for treatment or tests.
- To refuse to talk or see anyone not officially connected with English Plastic Surgery Center & Surgi-Spa.
- To have your identity remain completely confidential.
- To decide who has knowledge of your being a patient at English Plastic Surgery Center & Surgi-Spa and deciding whom the staff may speak to.
- To know of any persons having access to your medical records and the person(s) viewing them.
- To have certain information within your medical records withheld when giving to requesting parties within what the law allows.
- To express your opinions, recommendations and complaints in relation to policies and services offered by English Plastic Surgery Center & Surgi-Spa without fear of discrimination.
- To receive a detailed explanation of your total bill for the services provided English Plastic Surgery Center & Surgi-Spa.
- To express any grievance to the management of English Plastic Surgery Center & Surgi-Spa or if necessary, Dr. English.
- To consent or decline to participate in proposed research studies and treatment requiring direct patient involvement, and to have those studies fully explained prior to consent.
Responsibilities
In addition to rights as a patient at English Plastic Surgery Center & Surgi-Spa you also have these responsibilities:
- To provide information about past illness, hospitalization, medication and other matters relating to your health in order to provide the best possible treatment.
- To cooperate with all staff at English Plastic Surgery Center & Surgi-Spa and to ask questions if directions and/or procedures are not understood.
- To be respectful of the property of other persons and the property of English Plastic Surgery Center & Surgi-Spa.
- To follow all instructions and medical orders by Dr. English and his nurses in order to enable them to properly provide you with medical care.
- To have a responsible person available to English Plastic Surgery Center & Surgi-Spa for review of your treatment and care in case you are unable to properly communicate with Dr. English or his nurses.
- To pay all bills for medical services in a timely manner, or by being personally responsible for payment of any medical services two weeks prior to any procedure(s).
- To not complicate or endanger your ability to get well by consuming alcohol and toxic substances while under the care of English Plastic Surgery Center & Surgi-Spa.
- To call with ANY questions concerning you pre and post operative care.
- To provide English Plastic Surgery Center & Surgi-Spa with any information, old or new, in order to bill your insurance company for services rendered.
It is our desire to make your patient care as pleasant as possible. If you should have a question regarding our policies or procedures, please feel free to ask our personnel for an explanation. If at any time you experience a problem with our services, we want to know. Your comfort is very important, and we will gladly see that any problem you have is corrected if possible. We want to provide you with the very best possible service. If your needs are not being met, please let us know.
